We are contracted with the following insurances. We are considered out-of-network providers for some vision plans for our optical hardware services. Please contact our friendly staff for more information on your coverage.
Most of these are covered by one of the contract networks listed above. We advise you to call your insurance company to confirm our network participation status before your appointment. Companies commonly using one of the above networks include:
If you have an insurance listed above, we will collect any applicable co-payments and charges for non-covered services from you at the time of service. We will file a claim for services and expect to be reimbursed by your insurance company. Any amounts not paid by your insurance will be billed to you. Please be aware that insurance payments cannot be guaranteed, and your benefits are not determined until your claim is processed by your insurance company. You are financially responsible for any services your insurance company does not pay.
We are acknowledged as out-of-network providers for some known vision plans. We will provide you with the paperwork necessary and help you complete your claim for reimbursement from your vision plan. While most of these companies will reimburse you directly from your out of network benefit, some of these plans will pay directly to us your out of network allowances. For these out of network plans, you are responsible for payment in full at the time of service. We will refund you any amounts paid to us by your insurance company. Plans known to be out of network include EyeMed, Davis Vision, BlueVision, Spectera, Avesis, and Superior Vision.
If you have an insurance not listed above:
If you have insurance benefits provided by a company not listed above, we will collect your balance at the time of service and file a claim with your insurance company on your behalf. After your claim is processed, any insurance payment will be reimbursed directly to you.
NOTES:
1. Many patients without routine vision coverage have their exams or office visits covered under their medical insurance because of a medical condition such as allergies, diabetes, dry eyes, cataracts, glaucoma, macular degeneration, or other eye or other medical health conditions.
2. Many people will have separate routine vision plans or vision insurance companies from their medical insurance. It is important to note that we may be a preferred provider for one or the other companies but not necessarily both. We are not responsible for determining our provider status before billing services to your insurance company.
